New look for training. This is a different design from the summer camp gear, so every athlete gets a new shirt and bag even if they came to camp.

Acknowledgments

A few things we need you to confirm.

Liability Waiver. I have read and agree to the Rising Stars Academy liability waiver on behalf of my athlete.* Code of Conduct. I have reviewed the athlete code of conduct and my athlete agrees to follow it.* Pickup Policy. I understand that anyone not listed as a parent or approved pickup must be given permission by me before they can pick up my athlete, and that I'll be there within 15 minutes of each session or scrimmage ending.* Faith-Based Program. I understand Rising Stars Academy is a Christian program and sessions may include prayer and short devotionals. Photo & Video Release. I give permission for my athlete to be photographed or filmed during training and scrimmages for use on the Rising Stars Academy website, social media, and promotional materials. (Optional. Leave unchecked if you'd prefer.)
